HIPAA Notice of Privacy Practices

Our duties

We are required by law to maintain the privacy of your protected health information (PHI), provide you this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.

• Payment — to process eligible charges, including FSA/HSA where applicable.
• Operations — to run and improve our practice.

Other uses and disclosures will be made only with your written authorization, which you may revoke at any time.

• Inspect and obtain a copy of your records
• Request corrections to your records
• Request restrictions on certain uses and disclosures
• Request confidential communications
• Receive an accounting of certain disclosures
• Obtain a paper copy of this notice on request

Breach notification

We will notify you if a breach occurs that may have compromised the privacy or security of your PHI.

Complaints

You may complain to us or to the U.S. Department of Health and Human Services if you believe your privacy rights have been violated. We will not retaliate against you for filing a complaint.
